    The gap is rarely intent. It is visibility, ownership and evidence — each addressable once understood, and each now held to a higher standard by data protection law, sector requirements and the EU AI Act. These are the areas worth examining before a decision has to be explained to a board, a regulator or a court.

    AI tools can be adopted outside formal approval processes — through individual initiative, embedded product features or procurement — creating visibility and control gaps, including where personal or sensitive data may be involved.

    Without a current view of where AI influences decisions, risk cannot be judged proportionately: exposure may be over-estimated in low-risk areas and missed entirely in the places that matter most.

    Where ownership of AI-influenced decisions is not explicit, it can be unclear who is answerable, on what basis the decision was made, and what record exists — so responsibility is assumed rather than held.
